Active Horizens is an independent, unregistered NDIS provider supporting self-managed and plan-managed participants.

We currently work with self-managed and plan-managed NDIS participants. We are not able to support agency-managed (NDIA-managed) participants at this time.

Community Access is our core hourly support. Fitness, sport and aquatic programs are available as additional specialised services by enquiry.

Ways we support you

Start with Community Access & General Support. If you want additional specialised support — personal training, group fitness, or sport and aquatic programs — those sit underneath and are priced on enquiry.

Supporting participants to access their community, attend appointments, enjoy recreational activities, stay active and build independence.

Minimum 3 hoursSessions are booked for a minimum of three hours.

Travel$0.99 per kilometre.

Parking & tollsCharged at cost when they apply.

Rates are subject to review in line with NDIS Pricing Arrangements and Price Limits. Participants are notified of any changes before they take effect.

Clear about how we work

Unregistered providerActive Horizens is an independent, unregistered NDIS provider supporting self-managed and plan-managed participants. We currently work with self-managed and plan-managed NDIS participants. We are not able to support agency-managed (NDIA-managed) participants at this time.

Screening & checksNDIS Worker Screening Checks and Working with Children Checks where required. First Aid and CPR certification held by our team.

Insurance & recordsPublic liability and professional indemnity insurance, progress notes and secure record keeping.

SafeguardingRisk assessments, incident management, emergency response procedures and clear support boundaries.

Privacy & consentPrivacy handled under the Privacy Act 1988 (Cth). Photo and video use only with prior written consent, withdrawable at any time. See our privacy notice.

ComplaintsA clear complaints process with external escalation options. See our complaints process.

We communicate extra costs such as venue entry, parking or tolls with you, your parent or guardian before they apply. Agreements can be ended with two weeks’ written notice.
