These draft terms apply to use of the Active Horizens website. Active Horizens is an independent, unregistered NDIS provider supporting self-managed and plan-managed participants.
Information on this site
Service descriptions and rates (including Community Access hourly rates) are guides. Rates are subject to review in line with NDIS Pricing Arrangements and Price Limits. Participants are notified of any changes before they take effect. We confirm current rates and suitability with you before support starts. Minimum session length, travel and other costs are described on our services page.
Enquiries and preferred times
Submitting the contact form is not a booking confirmation. Preferred days and times are requests only until we confirm availability with you.
Who we support
We support self-managed and plan-managed NDIS participants. We are not a registered NDIS provider.
Liability
Website content is provided in good faith for general information. Final service terms will be set out in a service agreement with you. This draft is not a substitute for that agreement.
